The person who wrote the comment still sees it.

Open their own Instagram, go to your post, and there it is. Normal. Nothing marked, nothing greyed out.

From their side nothing happened at all.

That's deliberate. If the app told people their comment was filtered, they would simply post it again without the filtered word.

So the design hides the hiding. It's the same logic behind restricting an account.

Who can and cannot see a hidden comment

Worth listing plainly, because this is the actual question.

You can see it, in your hidden folder, if you go looking.

The person who wrote it sees it, in place, as if nothing happened.

Everybody else sees nothing. No gap, no placeholder, no comment count difference that would give it away.

So a hidden comment is invisible to the audience it was performed for, which is usually the point of the comment in the first place.

Hiding is not deleting

Two different actions and they behave differently.

Deleting removes the comment for everyone, including the person who wrote it. They can notice that, because they go back and it's gone.

Hiding leaves it in place for them and removes it for everyone else. Nobody notices anything.

Flip the question around, because it's the one people usually arrive with.

If you commented on somebody's post and nobody replied, your comment may be sitting in their hidden folder.

You would have no way to tell. It looks completely normal to you.

The only test is to check the post from a different account, or logged out, and see whether your comment appears.

Public posts can be checked without logging in at all, which is what the viewer is for.

Hidden Words does more than comments and this half gets ignored.

It filters message requests from people you don't follow, moving anything matching your list into a separate folder.

Instagram also filters requests it thinks are spam, whether or not you set any words.

Setting it up without overdoing it

Two mistakes people make on their first go.

The first is adding words that are too short. A three letter word will match inside longer words and filter comments you wanted.

The second is adding so many words that ordinary conversation stops appearing, and then wondering why engagement fell.

Start with Instagram's own default offensive list, which is a switch rather than a typing job.

Then add your own words only when you actually see a pattern. Ten well chosen words beat a hundred guessed ones.

If somebody compares your post on two devices, one logged in as them and one not, they will see their comment on one and not the other.

Nobody does this by accident. It requires already suspecting.

There's also the case where they ask a friend whether the comment is visible, which does happen.

So treat it as quiet rather than undetectable. Almost everything on Instagram is quiet rather than undetectable.

Should you use it?

Yes, if you post publicly at all.

The default offensive filter costs you one tap and removes a category of comment you do not want your followers reading under your photo.

It also removes the moment where you decide whether to respond, which is the part that actually takes the day out of you.

You never have to open the folder. That's the design.

If you post rarely and to a private account, it changes very little and you can skip it.
